“Formstack is in such a state of growth right now as we expand into new markets and new product categories, and with that growth, it’s imperative that we bring in the right people with the right experience to help us continue on that journey,” said Chris Byers, CEO of Formstack. “We have put so much intentional effort into creating an exceptional employee-first culture, so bringing in a proven HR leader like Tammy, with more than 26 years of experience, to help us continue to foster and cultivate our award-winning culture was an easy decision. Tammy’s expertise will be an integral piece of our growth plans at Formstack and we’re looking forward to seeing her take our company culture to the next level.”

Polk will lead the acquisition, growth and retention of Formstack employees. Before joining Formstack, Polk spent more than 26 years growing talent at organizations of all sizes both public and private. Throughout her career, she has earned results by balancing strategic planning and operational execution and has a track record of scaling productive teams quickly and building lasting relationships. She will also be responsible for continuing to expand on Formstack’s already unique employee benefit offerings, such as step-up bonuses, wellness and technology stipends, free weekly lunches among many more.

“Over the course of my HR career, I’ve seen a variety of strategic approaches to creating company culture, and I can honestly say that what Formstack has built, in terms of putting their employees first, has been impressive,” said Polk. “The exceptional culture at Formstack was in place long before it became a necessity to retain employees and it’s obvious that this is a culture that truly listens to what its employees need instead of just checking a box. I can’t wait to join the team and use my experience and passion in HR to continue building upon Formstack’s already strong foundation.”

The executive announcement follows the company’s recognition in the Financial Times ranking of The Americas’ Fastest Growing Companies, nominations for two TechPoint MIRA Awards, and inclusion in 2022’s Best Places to Work in Indiana. Formstack has seen huge growth following a $425M investment at the end of 2021 and plans to grow more than 160% in the next three years, with expansion into new verticals and international markets. The company also increased employee headcount by 66% in the past three years, with sights set on adding another 30% over the next three years. In addition to Polk’s executive appointment, Formstack recently appointed two other members to the executive team, Liza Smyth as senior vice president of customer experience and Kristine Camron as senior vice president and general counsel. Formstack is currently hiring for positions across the company.
